Key Responsibilities:

- Educate patients and their families on home exercise programs and techniques to ensure continuity of care.

- Document patient progress and modify treatment plans as necessary to achieve optimal outcomes.
Qualifications:
- Valid state licensure as a Physical Therapist.
- Proven experience in home health or a related healthcare setting is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
