Job description:
The Care Management Services Director is responsible for the strategic planning, organization, and management of patient care facilitation and transitions within a hospital setting. The role oversees case management, social work, utilization management, and post-acute services coordination to ensure optimal patient flow and quality care.
Key Responsibilities- Develop, implement, and oversee hospital-wide case management programs to enhance patient transitions, care coordination, and overall patient flow.
- Lead and manage a multidisciplinary team, ensuring alignment with hospital-wide patient experience goals.
- Improve case management processes, ensuring compliance with regulatory requirements and best practices.
- Optimize resource utilization through effective discharge planning, utilization review, and post-acute care coordination.
- Collaborate with physicians, nurses, and healthcare providers to streamline care transitions and minimize patient readmissions.
- Drive key performance metrics, focusing on patient outcomes, cost management, and operational efficiency.
- Ensure strong financial stewardship by implementing cost-effective patient management strategies.
- Maintain and promote effective communication channels between leadership, clinical staff, and external stakeholders.
Qualifications:
Must-Haves:
- Current RN License (or eligibility to obtain).
- 5+ years of case management experience in a hospital or healthcare setting.
- Strong communication and customer relations skills.
Nice-To-Haves:
- Masters Degree in Healthcare Administration, Nursing, or related field.
- 3-5 years in a supervisory or leadership role.
