Job description:
*Reports to Nurse Manager
Job Summary:
Responsible for day-to-day supervision of direct patient care, management of 3rd party payer authorizations, managing the weekly schedules of clinicians, taking the referral intakes, oversight of the clinical record reviews and participating in the monthly QA activities.
Responsibilities/Essential Functions:
- Intake & assignment of patient to field staff.
- Follows up on referrals by contacting patients and takes doctors orders (day-to-day patient care management).
- Documents telephone and in-office conferences with physicians, patients/family, nursing staff, and significant others.
- Takes and reports to doctor all test results of lab. Etc., performed by agency staff.
- Manages the case conference process.
- Coordinates all services provided to patients.
- Manages the HR needs of the clinical department, notifying the Nurse Manager when new hires are needed to stay proactive with growth plan/
- Assists with interviews and hires select home healthcare personnel.
- Orients professional staff to clinical policies and procedures of our agency. Consults with physicians when necessary to ensure and maintain safe, efficient, quality patient care.
- Supervises all professional and non-professional staff to ensure quality performance standards.
- Establishes mutual goal setting and achievement standards.
- Projects a concerned, professional appearance toward departmental personnel.
- Maintains high visibility and availability via telephone.
- Works to develop a positive rapport/relationship with all staff members.
- Performs tasks as delegated by Nurse Manager.
- Shares on-call responsibility on an equitable basis with other nursing staff.
- Actively develops and pursues referral sources, as well as facilitates on-going relationships with community resources affiliated with home care services.
- Participates in agency QA programs.
- Participates in in-service programs.
- Monitor utilization, productivity, and tracks patient outcomes.
- Ensures compliance with Agencys policy and procedures.
- Participates in on-call rotation.
- Provides coding and OASIS support to admitting clinicians.
- Provides direct patient care as staffing requires.
- Assesses the ability of the caregiver to meet patients immediate needs upon admission and throughout care.
- Completes daily chart audits.
- Perform OASIS and 485 Review.
- Ensures timely submission of visit notes and Oasis.
- Perform field supervision.
- Ensure consistency of 5-star ratings in CMS Compare.
- Other duties as assigned.
Qualifications:
Qualifications:
- Must be a registered nurse, licensed in CT.
- Must have at least two (2) years nursing experience and a background in public health, home health, pr hospital work, including 1-year supervisory experience.
- Must have criminal background check and current CPR certification.
- In-field supervision 1-2 times a week
- On-call rotation
