Responsibilities- Conduct comprehensive patient assessments to determine healthcare needs.
- Develop individualized care plans in collaboration with patients and families.
- Administer medications and treatment as prescribed by physicians.
- Monitor and document patient progress and response to treatment.
- Educate patients and caregivers on disease management and health maintenance.
- Coordinate with interdisciplinary teams to ensure comprehensive patient care.
- Respond to patient inquiries and provide support in a timely manner.
Qualifications:
- Active (unrestricted) nursing license in the State of California
- 1 year(professional)nursing experience in Home Health or Acute Care
- Valid Driver's License w/ Clean DMV
