Admissions Field RN Hospice
Responsibilities:
- Monday through Friday
- Admissions of Hospice patients within 48 hours as per physician's orders
- Responsible for providing the initial assessment and admission for patients referred for Hospice Services.
- Act as a Liaison with referring institutions for transition of care
Requirements:
- A Registered Professional Nurse with current License
- Hospice agency experience required
- Knowledge of hospice regulations and qualifications for hospice admission.
- Associates Degree; Bachelor’s degree preferred
